Why does COVID-19 trigger metabolic issues?

Western medicine is still mapping the exact pathways, but the current consensus points to three main mechanisms:

  1. ACE2 Receptor Dysregulation The SARS-CoV-2 virus enters cells via the ACE2 receptor. These receptors are not just in the lungs; they are heavily concentrated in the liver, pancreas, and blood vessels. When the virus binds to these receptors, it disrupts the Renin-Angiotensin System (RAS), which is responsible for regulating blood pressure. This often leads to vasoconstriction and increased blood pressure.
  2. Systemic Inflammation & Lipid Metabolism The “cytokine storm” or chronic low-grade inflammation associated with Long COVID can interfere with how the liver processes lipids. Inflammation increases the production of VLDL (very-low-density lipoprotein) and decreases the clearance of LDL and triglycerides. Essentially, the body stays in a “pro-inflammatory” state that keeps fat levels high in the bloodstream.
  3. Endothelial Dysfunction COVID-19 is increasingly viewed as a vascular disease. The virus damages the lining of the blood vessels (the endothelium). When the endothelium is damaged, the body may increase cholesterol production as a “patching” mechanism, and the vessels become stiffer, contributing to hypertension.

Part 2: Clinical Case Study

Patient Profile

  • Patient: A, 35-year-old University Lecturer.
  • Primary Complaint: Severe post-viral fatigue following a COVID-19 infection.
  • Clinical Findings (Western): Blood tests revealed significantly elevated Triglycerides and LDL Cholesterol levels, exceeding laboratory limits.

Traditional Chinese Medicine Diagnosis

Despite the Western diagnosis of dyslipidemia, TCM looks at the whole picture of the body. Mr. A presented with:

  • Tongue: Red body (Heat) with a thick, yellow, greasy coat (Damp-Heat).
  • Pulse: Slight rapid (Heat) and wiry (Liver involvement/Stress).
  • Pattern Diagnosis: Damp-Heat in the Liver and Gallbladder and blood stagnation. In TCM, the Liver and Gallbladder are responsible for the smooth flow of “Qi” and the processing of “fats” (phlegm/dampness). When heat and dampness “steam” these organs, metabolic waste (cholesterol) accumulates which cause blood to flow slowly.

Treatment Strategy: A Dual Approach

The goal was to clear the “sludge” (Damp-Heat) and restore the “engine” (Spleen/Digestive function).

  1. Acupuncture and cupping (2 Sessions)
  • Focus: Tonify the Spleen to transform dampness and clear the Liver/Gallbladder.Cupping on the back to move the stagnation.
  • Key Points: Spleen 6 (to move blood), PC6, ST40 (to resolve phlegm/lipids) and LV3/GB34 (to clear Liver heat).
  1. Herbal Medicine
  • Formula: Modified Xiao Chai Hu Tang .
  • Modifications: * Heat-clearing herbs: Huang Qin and Huang Lian to address the red tongue and rapid pulse.
    • Blood-moving herbs: Chuan Xiong, Dan Shen, Dang Gui to address the “Wiry” pulse and potential micro-clotting/stagnation common in Long COVID.
    • Damp-draining herbs: Long Dan Cao , Shan Zha to physically lower the triglyceride “load” as seen in the yellow greasy tongue coat.

Clinical Outcome

After two batches of herbal medicine and two acupuncture sessions, the patient underwent follow-up blood work.

  • LDL Cholesterol: Returned to normal range.
  • Triglycerides: Returned to normal range.
  • Subjective Improvement: Significant reduction in fatigue and improved energy levels.

This case illustrates that post-COVID metabolic changes aren’t just “numbers” on a lab report; they are often the result of internal “Damp-Heat” and “Stagnation” left behind by the viral battle. By treating the underlying TCM pattern, the Western markers (cholesterol/BP) often stabilize naturally.
